Subject: Advanced database design for storing questions/answers for multiple applications.

My question is how can the �Insurance Applications Data Model� model could be modified so depending on
the answer the client/customer gives could have a follow-on set on new question which is relevant to the answer.
i.e. If the customer answered �yes� to �Do you smoke?� Follow up question will then ask �For how long and how much per day?�

My objective is to develop a web form where the customer answers the minimum number of questions and only answer question
which are relevant. Hope that makes sense.
