Data Model for Visits to a Church Food Pantry

A. The requirement is to design a Database.
Here is the original Question :-

Good Morning,

My church is looking for a data base to keep track of visitors to our Food Pantry, which operates once a month.
I have been through your list and did not see anything for food pantries.

If I missed it, please let me know.

We keep a manual list consisting of name, address, ZIP Code, gender, phone number, DOB, count of household members by age bracket,
and list of types of Assistance being received.  We then send this list to our local Food Bank.

Most of the data is constant from one visit to the next.

If I asked for the Access database for 'Patients Visits to the Doctor', can it be modified?
This model seemed like it might be the closest to what we need, as it contains basic patient info and multiple visits.

Also, do I need Access to use your model and/or to modify it?
