the following style :-
1) The PATIENT makes at least one APPOINTMENT
2) An APPOINTMENT can be Cancelled, Kept, or Planned(in the future).
3) The PATIENT has certain DESIRED_OUTCOMES
4) The PATIENT is put on a specified DIETPLAN.
5) Each DIETPLAN has specified EXPECTED_OUTCOMES
6) LAB_TESTS are carried out for a PATIENT.
7) The LAB_TEST_RESULTS become a part of the PATIENT_HISTORY.
8) Etc..
9) Etc..
